Autism Testing for Women
There are biases inherent in Autism testing for women, that result in girls and women being underdiagnosed or misdiagnosed. The standard thinking on autism has been that a diagnosis of autism primarily presents in boys and men with the ratio of men to women being 4:1. There is currently no autism testing for women that uses biomarkers, so diagnosis is based on behavioral expression of core characteristics including the presence of social communication difficulties, impaired nonverbal behaviors, persistent interests, repetitive behaviors, sensory sensitivities, and difficulty adapting to changes in routine. Differences in diagnosis across gender are now thought to be due to differences in how the core characteristics of autism are expressed. Some of the same characteristics may be expressed differently based on gender.
Autistic girls and women have more social relationships which makes it appear that they do not struggle with social communication difficulties that interfere with relationship building. But autistic girls and women tend to have more conflict in their relationships, something that has recently been acknowledged. Some core characteristics look different in girls and women. They may have less intense repetitive behaviors, like twirling their hair instead of rocking back and forth.
Persistent interests look different in girls and women as well. They often have interests that are considered more socially acceptable, like an interest in recipes that initially does not seem to be more than a hobby. On closer examination, the woman who has an interest in recipes might have a collection of 2000 recipes that she files and organizes. She may be fluent in the biology of the herbs she uses, and may talk at length about recipes throughout history, share details of famous culinary dishes in different countries, and have in-depth knowledge about the preparation of food. Her interest may keep her up at night looking at recipes or researching the history of breakfasts across the world. She may avoid household chores, look up recipes and facts at work, and avoid socializing to engage in her persistent interest.
